Job summary

Centene Corp. is seeking a LTSS Service Care Coordinator to support aging and physically disabled members in Chicago. This hybrid-remote role involves traveling to member homes up to 75% of the time to assess needs and coordinate services.

You will help develop and monitor long-term care plans, connect families with providers and community resources, and educate members on benefits and options. A Bachelor’s degree and relevant experience are required.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• Illinois Plan Only: RN licensed in Illinois or Bachelor/Master’s in human services field or LPN with experience.

Responsibilities

• Evaluates member needs and coordinates long-term care plans.
• Identifies providers and community resources for long-term care.
• Coordinates between member/family and care team to ensure services are accessible.
• Provides education to members and families on procedures, options, and benefits.
• Monitors care plans and adjusts based on member needs.
• Documents and maintains member information for regulatory compliance.
