Centene is seeking a care coordination professional to support aging and disability waivers in Illinois. The role is hybrid with substantial home visits, traveling to member locations, and partner coordination to meet patient needs.

The position requires a Bachelor's degree and related experience, with eligibility criteria including licensure or advanced human services education. Training runs on a Monday–Friday schedule with ongoing field work and collaboration with care teams.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• RN license in Illinois, or Bachelor/Master's in human services, or LPN with one year experience.

Responsabilités

• Evaluates member needs and resources to recommend the best outcomes.
• Assists in developing long-term care plans and coordinates with providers.
• Coordinates between members, families, and care teams to ensure access to services.
• Provides resource support for employment, housing, independent living, and related needs.
• Monitors plans and outcomes and recommends adjustments based on needs.
• Interacts with long-term care providers to meet member needs.
• Maintains member information to ensure regulatory compliance.
• May perform on-site visits and collaborate with providers or community resources.
• Provides education on procedures, options, and benefits.
• Provides feedback to leadership to improve quality and efficiency.
• Performs other duties as assigned.
• Complies with all policies and standards.
