Job summary

Centene Corporation seeks a care management professional to assist in developing and coordinating holistic care plans for aging and disabled members. The role involves hybrid-remote work with 50% travel to member homes and requires prior case management or home-visit experience, preferably within 25 minutes of Huntley, IL.

Primary duties include evaluating member needs, coordinating services, and educating families about benefits. Requires a Bachelor's degree and 1 year of related experience.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• For Illinois Plan Only: RN licensed in Illinois, or Bachelor/Master’s in human services related field, or LPN with 1 year experience.

Responsibilities

• Evaluates the needs of the member, resources available, and recommends the plan for best outcome.
• Develops ongoing long-term care plans/service plans and identifies providers or community resources.
• Coordinates between member/family/caregivers and care provider team to ensure services are accessible.
• Provides resource support to members and families for needs (employment, housing, independent living, etc.).
• Monitors care plans and outcomes and provides recommendations based on needs.
• Interacts with long-term care providers to ensure member needs are met.
• Collects and maintains long-term care member information for regulatory compliance.
• May perform on-site visits to assess member needs and collaborate with providers.
• Provides education to members and caregivers on procedures and options.
• Gives feedback to leadership on improving quality and cost-effective service delivery.
