Centene is seeking a Hybrid Home-Based Care Manager in Illinois to develop and coordinate holistic long-term care plans for aging and disabled members. The role involves education for members and families, collaboration with care teams, and ensuring access to services.

Travel is heavy (75%), with training Monday–Friday 8-5 CT; candidates should live within 30-40 minutes of Des Plaines. Requires Bachelor's degree and 1 year of related experience; preference for RN/LPN or Human Services degree.

Qualifications

Bachelor's degree and 1 year related experience required.
RN licensed in Illinois preferred.
Bachelor or Master’s in Human Services field acceptable.
LPN with 1 year experience acceptable.

Responsabilités

Evaluate member needs and resources to plan best outcomes.
Develop long-term care plans and identify providers and resources.
Coordinate between member/family and care team to ensure services are accessible.
Provide resource support for employment, housing, independent living, etc.
Monitor plans and outcomes and provide recommendations.
Interact with long-term care providers and partners.
Document and maintain member information for compliance.
May perform on-site visits to assess needs and collaborate with providers.
Educate members and families on procedures, options, benefits.
Provide feedback to leadership to improve care delivery.
Perform other duties as assigned.
Comply with policies and standards.
