Job summary

Centene Corporation is seeking a Care Manager to support community-based members with aging and disability waivers in a hybrid-remote role. It involves 75% travel to member homes and coordination with care teams to ensure accessible, quality services.

The role requires a Bachelor's degree and 1 year of related experience, with preference for candidates near Des Plaines, IL and experience in case management or advocacy. Training follows standard CT hours.

Qualifications

• Bachelor's degree and 1 year of related experience.
• Preference for past case management, advocacy or home visits experience.
• Reside within 30–40 minutes of Des Plaines, IL.
• Hybrid-remote role with 75% travel to member homes.

Responsibilities

• Evaluates the needs of the member, resources available, and recommends/coordinates the plan for the best outcome.
• Assists with developing long-term care plans and identifies providers, specialists, and community resources.
• Coordinates between member/family and care team to ensure services are accessible.
• Provides resource support for needs (housing, employment, independent living, etc.).
• Monitors care plans and outcomes and provides recommendations based on member needs.
• Interacts with long-term care providers and partners to meet member needs.
• Collects and maintains member information to ensure regulatory compliance.
• May perform on-site visits to assess member needs and collaborate with providers.
• Delivers education to long-term care members and families on procedures and benefits.
• Provides leadership with feedback to improve quality and service delivery.
