Job summary

Tuesday Health seeks an LPN to deliver coordinated, compassionate care to members with serious illnesses in a hybrid setting. You will work with nurse practitioners and RNs to administer assessments, support care planning, and engage members using Tuesday Health’s electronic systems.

Strong communication and empathy are essential. Responsibilities include participating in rounds, prioritizing member needs, and building trusting relationships to ensure a positive, person-centered care journey.

Qualifications

• Active and unrestricted LPN license in the State of Ohio.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model.
• Strong communication and organizational skills.

Responsibilities

• Administer multiple assessments and screening tools with the care team.
• Prioritize needs based on assessments and inform the interdisciplinary team.
• Be accountable for care plan development within the team.
• Participate in rounds to ensure coordinated member experience.
• Learn Tuesday Health electronic systems and tools.
• Build trusting relationships with members through listening and empathy.
• Act as quarterback of the multidisciplinary team transforming care.
