Job summary
Tuesday Health in Houston is seeking a Complex Care Navigator LVN to coordinate assessments, care plans, and delivery with nurse practitioners and RNs.
You will use our electronic tools to optimize the member experience, participate in rounds, and build trusting, empathetic relationships.
Texas LVN license and strong communication are required; remote/hybrid work with occasional travel is offered.
Qualifications
- Active and unrestricted LPN license in Texas.
- Experience in clinical/medical settings preferred.
- Experience in a multi-disciplinary care model across providers and care management.
- Strong communication and organizational skills.
- Ability to multitask and adapt to rapid changes in the business.
Responsibilities
- Administer multiple assessments and screenings within the clinical model with support from NP/RN colleagues.
- Prioritize needs based on assessments and coordinate with the interdisciplinary team.
- Be accountable for care plan development within the team.
- Participate in internal and joint rounds to optimize member experience.
- Learn the Tuesday Health electronic systems and tools to deliver coordinated care.
- Build trusting relationships with members through listening and empathy.
- Serve as quarterback of the multidisciplinary team to transform care for members.
