Tuesday Health seeks a Licensed Practical Nurse (LPN) in Massachusetts to provide coordinated, compassionate care for members with serious illnesses. You will work with nurse practitioners and RNs to perform assessments, prioritize needs, and develop individualized care plans within a multidisciplinary team.

The role emphasizes trust, empathy, and strong communication, with remote work options and occasional onsite sessions as part of a patient-centered care model.

Qualifications

• Active and unrestricted LPN license in MA with no board actions.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care models and cross-provider collaboration.

Responsibilities

• Administer clinical assessments and screenings within the care model.
• Prioritize member needs based on assessments and coordinate with the care team.
• Develop and execute individualized care plans with the multidisciplinary team.
• Participate in care rounds and ensure aligned, patient-centered communication.
• Learn and navigate Tuesday Health's electronic systems to optimize care.
• Build trusting relationships with members through listening and empathy.
