Job summary

Qualifications

• Active and unrestricted LPN license in Massachusetts.

• Ability to navigate documentation systems and learn new tools.

Responsibilities

• Administer multiple assessments and screenings with team support.
• Prioritize needs based on assessments and coordinate with care team.
• Develop and implement care plans within the multidisciplinary team.
• Participate in rounds to ensure coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
