Job summary
Tuesday Health in Peabody, MA is seeking a Licensed Practical Nurse to join our Complex Care Navigator team. You will work with nurse practitioners and RNs to administer assessments, support care plans, and ensure a compassionate, person-centered experience for members with serious illness.

Qualifications
- Active and unrestricted LPN license in Massachusetts.
- Clinical/medical experience preferred.
- Experience in multidisciplinary care model.
- Strong communication and organization skills.
- Ability to multitask and maintain calm under pressure.
- Valid driver''''s license and insurance.
- Willingness to travel for onsite sessions.
Responsibilities
- Administer assessments and screenings with team support.
- Prioritize needs and help develop care plans.
- Participate in rounds to coordinate member care.
- Learn and use Tuesday Health electronic systems.
- Build trusting relationships with members.
- Act as quarterback of multidisciplinary team.
