Tuesday Health is hiring a Licensed Practical Nurse (LPN) to join our complex care team in Massachusetts. The LPN will work with nurse practitioners and RNs to perform assessments, prioritize needs, and help implement care plans that improve quality of life for patients with serious illness.

This role emphasizes compassionate care, coordination within an interdisciplinary team, and proficiency with electronic health records to support seamless member experiences.

Qualifications

• Active and unrestricted LPN license in Massachusetts.
• Experience in clinical/medical setting preferred.
• Strong communication and organizational skills.

Responsibilities

• Administer clinical assessments and screenings.
• Prioritize member needs based on assessments.
• Develop and execute individualized care plans within the MDT.
• Participate in care rounds to optimize member experience.
• Learn and use Tuesday Health electronic systems.
