Job summary

Tuesday Health, LLC seeks an LVN to deliver coordinated, compassionate care for members with serious illnesses. The LVN collaborates with NPs and RNs, administers assessments, and helps develop care plans across the care journey.

Responsibilities include participating in rounds, using electronic systems, and building trust with members. Strong communication and organizational skills are essential.

Qualifications

• Licensed Practical Nurse (LPN) in Texas, not restricted.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care models.
• Strong communication and organization skills.
• Ability to multitask and work under pressure.

Responsibilities

• Administer assessments and screenings with NP/RN support.
• Prioritize needs based on assessments and coordinate with care team.
• Develop and execute individualized care plans.
• Participate in care rounds to optimize member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trusting relationships with members and families.
• Serve as quarterback of the multidisciplinary care team.
