Qualifications

• Bachelor's degree or higher in a health-related field.
• RN experience in hospice/home health.
• Knowledge of Medicare Conditions of Participation and regulations.
• Proficiency with Outlook, Word, and Excel.

Responsibilities

• Lead and mentor the hospice care team to ensure high-quality patient care.
• Coordinate patient and family needs with interdisciplinary team.
• Oversee care plans, evaluations, and appropriate treatments.
• Monitor care delivery for Medicare and hospice regulatory standards.
• Review and approve care plans in EMR with accuracy.
• Participate in quality improvement and outcome reviews.
• Lead interdisciplinary team meetings with clinicians and medical director.
• Provide ongoing education and performance feedback to staff.
