Job summary

Cabrillo Hospice is seeking an experienced Hospice Registered Nurse Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices.

You will lead a compassionate team, manage care plans, and ensure Medicare standards are met while delivering end-of-life care in patients’ homes. A strong nursing background in hospice and HIPAA compliance is essential, with opportunities for growth and on-call leadership.

Qualifications

• RN with hospice or palliative care experience preferred.
• Knowledge of Medicare Conditions of Participation and regulatory standards.
• Proficiency with EMR, Word, Excel, and Outlook.
• Strong communication and mentorship abilities.
• Ability to lead interdisciplinary teams and coordinate care.

Responsibilities

• Lead and mentor hospice care team to maintain high standards of care.
• Coordinate patient and family care plans with interdisciplinary team.
• Oversee case management, evaluations, and treatments for quality outcomes.
• Provide clinical oversight and ensure regulatory compliance.
• Review care plans in EMR for accuracy and adherence to Medicare standards.
• Participate in quality improvement initiatives and outcome reviews.
• Lead team meetings with clinicians and medical director.
• Provide education and performance guidance for team members.
• Assist DCS in regulatory readiness and surveys.
• Participate in on-call leadership for 24/7 patient care oversight.
