Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, organize, direct, and provide hospice care in home and community settings. The role emphasizes nursing expertise and community health education.

You will lead the interdisciplinary team, develop patient plans of care, ensure OSHA standards, and support families while maintaining quality documentation. This full-time position offers a comprehensive benefits package and opportunities for growth.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current State RN license and CPR certification.
• Minimum two years of RN experience in clinical care, home health or hospice.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases to ensure third-party payer compliance.
• Supervise hospice aides and licensed practical nurses.
