Job summary

Cabrillo Hospice is seeking an experienced Hospice RN Case Manager to lead a care team and oversee patient plans in the Coronado region. You will mentor interdisciplinary staff, ensure regulatory compliance, and drive quality patient outcomes across home visits and clinic settings.

Responsibilities include coordinating care with patients and families, managing care plans in EMR, and ensuring adherence to Medicare Conditions of Participation.

Qualifications

• Bachelor’s degree in a health-related field or higher.
• RN with hospice or home health experience preferred.
• Strong knowledge of Medicare Conditions of Participation and regulatory standards.
• Proficiency with Outlook, Word, and Excel.

Responsibilities

• Lead and mentor the hospice care team, ensuring high standards of care.
• Oversee coordination with patients and families as team leader.
