Job summary

Cabrillo Hospice is seeking an experienced Hospice Registered Nurse Case Manager in Central LA. You will lead a multidisciplinary team, oversee care plans, and ensure regulatory compliance while delivering compassionate end-of-life care during home visits.

The role requires travel across multiple locations, on-call leadership, and mentoring of clinicians. Strong Medicare knowledge and EMR proficiency are essential for success.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Minimum 3 years of clinical RN experience with hospice experience.
• At least 1 year of supervisory or management experience in hospice.
• Strong knowledge of Medicare Conditions of Participation and regulatory standards.

Responsibilities

• Lead: supervise and mentor the hospice care team to maintain care standards.
• Coordinate patient and family care with interdisciplinary team members.
• Oversee care plans, evaluations, and treatments for optimal outcomes.
• Provide clinical oversight ensuring Medicare and hospice compliance.
• Review care plans in EMR for accuracy and compliance.
• Participate in quality assurance initiatives and drive improvements.
• Lead team meetings with clinicians and medical director.
• Offer ongoing education and guidance to team members.
• Support regulatory readiness with the Director of Clinical Services.
• Participate in on-call leadership to ensure 24/7 coverage.
