Cabrillo Hospice seeks an experienced Hospice RN Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices while delivering compassionate care to patients and families.

This role involves leadership, travel to multiple locations, 1–10 home visits daily, on-call rotation, and collaboration with the medical director to maintain regulatory readiness and quality improvement.

Qualifikationen

• Bachelor’s degree or higher in a health-related field.
• Clinical RN experience, hospice or home health preferred.
• Strong knowledge of Medicare Conditions of Participation and regulatory standards.
• Proficiency with Outlook, Word, and Excel.

• Lead and supervise hospice care team; mentor interdisciplinary staff.
• Coordinate patient and family care; provide direction to team members.
• Oversee care plans, evaluations, and treatments to optimize outcomes.
• Ensure compliance with Medicare Conditions of Participation and hospice standards.
• Review and approve care plans in the EMR with accuracy and adherence.
• Participate in quality improvement and data review initiatives.
• Lead interdisciplinary team meetings with clinicians and medical director.
• Provide education and documentation support for the team and orientation.
