Job summary

Cabrillo Hospice seeks an experienced Hospice Registered Nurse Case Manager to lead the care team, coordinate patient and family needs, and ensure compliant, high-quality end-of-life care. This role blends clinical oversight with mentoring of interdisciplinary staff and is pivotal in maintaining Medicare and hospice standards across all visits.

You’ll travel to patient homes, supervise care plans, document in EMR systems, and participate in regulatory readiness.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Active RN license in the state of practice.
• Experience in hospice care and case management preferred.

Responsibilities

• Lead and oversee hospice care teams to ensure high-quality patient care.
• Coordinate patient and family needs and ensure accurate care plans.
• Oversee case management, evaluations, and appropriate treatments.
• Ensure compliance with Medicare Conditions of Participation and hospice standards.
• Review care plans in the EMR and support documentation accuracy.
