Zusammenfassung

Affinity Hospice is seeking a Registered Nurse/Case Manager to plan, organize, direct and provide hospice care in home settings. The role emphasizes community health education and meeting patient and family needs through coordinated home services.

Full-time position requiring RN license, CPR, and 2+ years in clinical/hospice settings. Travel and on-call coverage are typical, with opportunities for advancement and end-of-life training.

Qualifikationen

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice preferred.

Aufgaben

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care with infection control measures.
• Assure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned.
