Qualifikationen

• Bachelor’s degree or higher in a health-related field.
• Clinical RN experience, hospice or home health preferred.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards.

Aufgaben

• Lead, mentor and supervise the hospice care team to maintain high standards of care.
• Coordinate patient and family care across interdisciplinary team members.
• Oversee care plans, evaluations, and treatments for optimal outcomes.
• Monitor compliance with Medicare Hospice Conditions of Participation and CMS standards.
• Review and approve care plans in the EMR with accuracy and HIPAA adherence.
• Participate in quality assurance initiatives and drive improvements.
• Lead interdepartmental team meetings with clinicians and medical director.
• Provide ongoing education and mentorship to staff and orientation support.
• Assist the DCS with regulatory readiness and surveys.
• participate in on-call leadership for 24/7 patient care oversight.
