Job summary

Anchor Health is seeking a dedicated Hospice Registered Nurse Case Manager to join our team in San Mateo County. The role focuses on comprehensive patient assessments, home visits, and care planning with patient and family involvement.

Responsibilities include administering meds, documenting care, and participating in interdisciplinary team meetings while supervising support staff. California RN license and CPR are required, along with a valid CA driver’s license and insured vehicle.

Qualifications

Excellent observation and communication skills.
Registered nurse in California with current CPR.
RN license and nursing degree from an accredited program.
Valid CA driver's license and insured vehicle.

Responsibilities

Completes initial, comprehensive and ongoing assessments to determine hospice needs.
Performs full physical assessments and reviews history of illness.
Carries out home visits and evaluates patient status.
Develops and revises the care plan with nursing diagnoses and patient/family input.
Administers medications/treatments as prescribed by physician.
Attends Interdisciplinary Group Meetings and documents care notes.
Supervises ancillary staff and delegates tasks as needed.
