Job summary

Cabrillo Hospice is seeking an experienced Hospice RN Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. Qualifications

• Bachelor’s degree or higher in a health-related field.
• RN experience with hospice or home health preferred.
• Knowledge of Medicare Conditions of Participation and regulatory standards.
• Proficiency with Outlook, Word, and Excel.
• Active BLS certification and valid driver’s license.

Responsibilities

• Lead and supervise hospice care team, ensuring high standards of care.
• Coordinate care plans with patients and families as team leader.
• Oversee case management, evaluations, and treatments.
• Monitor compliance with Medicare and hospice standards.
• Review and approve care plans in EMR with accuracy.
• Participate in quality assurance and performance improvement.
• Lead interdisciplinary team meetings with clinicians and medical director.
• Provide education and mentorship to hospice staff.
• Assist DCS with regulatory readiness and surveys.
• Participate in on-call leadership to ensure 24/7 oversight.
