Responsibilities

• Lead a team of Case Managers and monitor caseloads.
• Review referrals, screenings, and intake processes.
• Coordinate housing stability plans, move-in activities, and service referrals.
• Conduct client screenings to collect essential information for case planning.
• Maintain and secure client records and documentation in HMIS.
• Participate in outreach events and workshops as needed.
