Job summary

Qualifications

• Minimum of 2 years of experience as a case manager, working with homeless or vulnerable populations.
• Demonstrated leadership experience in supervising teams.

Responsibilities

• Assign and monitor team caseload and referrals.
• Conduct comprehensive client screenings and develop case plans.
• Monitor and audit case files for completeness and accuracy.
• Coordinate services and resources for veterans and families.
• Lead workshops and participate in outreach events.
