Job summary

Volunteers of America Los Angeles (VOALA) seeks a Case Manager Specialist I to provide intensive case management to eligible clients, meeting weekly. You are the primary contact to coordinate services, and you will handle a caseload across housing, health, education, and social supports.

On-site work is required; you will drive to client meetings and home visits, document plans, and advocate for clients before agencies.

Qualifications

• Associate’s degree in social work or related field.
• Completion of VOALA Case Management training within 6 months of hire.
• Fingerprint clearance and background checks required.
• Valid California Driver’s license with reliable vehicle.

Responsibilities

• Screen and assess clients to build a case plan.
• Provide service linkages and support to meet needs.
• Coordinate care and case management for caseload.
• Coordinate planning with clients to meet needs.
• Coordinate case conferences with clients and team.
• Drive to client meetings and home visits.
• Advocate for clients before agencies.
• Facilitate group education and document sessions.
