Job summary

Volunteers of America Los Angeles seeks a Case Manager Specialist I to provide intensive case management to eligible clients and coordinate services. You will meet clients weekly, assess needs, link services, and document progress.

On-site responsibilities include driving to client meetings and home visits, coordinating care plans, and advocating for clients with relevant agencies and service providers. VOALA offers competitive benefits.

Qualifications

• Minimum 1 year of general experience in case management or similar social services role.
• Associate’s degree or equivalent experience in social work or related field.
• Must complete VOALA Case Management training within 6 months of hire.

Responsibilities

• Conduits comprehensive screenings and assessments to develop a case plan.
• Provide service linkages and support systems to meet identified needs.
• Coordinate care, case management and housing navigation for assigned clients.
• Meet weekly with clients and drive to home visits when required.
• Advocate for clients before agencies and community bodies.
