Qualifications

Responsibilities

• Provide virtual care navigation for CKD/ESRD patients focusing on education and self-management.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs and barriers.
• Collaborate with nephrologists, care teams, and internal partners for coordinated patient care.
