Job summary

Fresenius Medical Care seeks a Kidney Care Specialist to educate CKD/ESRD patients, navigate care, and engage via field outreach and virtual support. You must live near the assigned territory and travel locally to meet patients and nephrology providers.

This role partners with nephrologists and care teams, documents in Salesforce/EMR, and supports value-based care initiatives while promoting timely education and patient-centered decisions.

Qualifications

• 1–3 years of experience in kidney care, chronic disease management, or related healthcare fields.
• Experience delivering virtual or telephonic patient education is strongly preferred.
• High school diploma or GED required; relevant licensure/certification preferred where applicable.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients using approved platforms.
• Conduct scheduled or on-demand virtual or in-person interactions to assess needs and preferences.
• Collaborate with nephrologists and care teams to support referrals and coordinated care.
