Job summary

Fresenius Medical Care is seeking a Kidney Care Advocate to navigate CKD/ESRD patients from education to care coordination, leveraging virtual and in-person interactions. You will liaise with nephrologists, care teams, and providers to close care gaps and drive patient-centered outcomes.

The role requires field-based activities across a defined territory, with strong emphasis on patient education, documentation, and cross-functional collaboration within the Kidney Care leadership team.

Qualifications

• High School diploma or GED required; 3–5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.
• Experience delivering virtual or telephonic patient education is strongly preferred.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients and discuss treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions.
• Collaborate with nephrologists and care teams to support coordinated patient care.
