Responsibilities

Conduct scheduled or on-demand virtual or in-person patient interactions.
Collaborate with nephrologists and care teams to coordinate referrals.
Build relationships with providers and support patient education updates.
Document interactions and outcomes in Salesforce/EMR systems; track engagement.
Support patient education materials development and improve workflows.
