Job summary

Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD patients and guide them through treatment options from diagnosis to informed decision-making. Qualifications

• High School diploma or G.E.D. required; 3-5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.
• Experience in kidney care or chronic disease management is desirable.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients using approved platforms.
• Conduct scheduled virtual or in-person patient interactions to assess needs and barriers.
• Collaborate with nephrologists and care teams for coordinated patient care.
• Build strong relationships with nephrologists and staff to support value-based care objectives.
• Ensure consistent messaging and patient follow-through across workflows.
• Document patient interactions and outcomes in designated systems (Salesforce, EMR).
• Support referral workflows by educating patients and tracking progress.
• Utilize dashboards and reports to track engagement and program performance.
• Stay up-to-date on kidney care and Fresenius Kidney Care programs.
• Participate in meetings, training, and performance reviews.
