Job summary

Fresenius Medical Care seeks a Kidney Care Specialist to educate CKD/ESRD patients, navigate care, and engage communities. You will travel regionally, meet nephrology providers, and support care coordination with virtual and in-person interactions.

Ideal candidates have 1–3 years in kidney care or related healthcare, strong communication, and experience with CRM/EMR tools. This role may be field-based, virtual, or hybrid, with varied market needs.

Qualifications

• High School diploma or GED; 3–5 years clinical background preferred.
• Licensure or certification preferred, as applicable.
• 1–3 years of experience in kidney care, chronic disease management, or related healthcare fields.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients and educate on treatment options.
• Conduct scheduled or on-demand virtual or in-person patient interactions to assess needs and barriers.
• Collaborate with nephrologists and care teams to support coordinated patient care and referrals.
• Build strong relationships with nephrologists, APNs, managers, care coordinators, and staff.
• Document patient interactions and outcomes in Salesforce, EMR, or approved platforms.
• Support referral workflows by educating patients and tracking follow-through.
• Utilize dashboards and reports to track patient engagement and program performance.
• Participate in development and improvement of virtual education materials.
