Job summary

Fresenius Medical Care’s Kidney Care Specialist role focuses on educating CKD/ESRD patients, driving early education adoption, and care navigation through field-based, virtual, or hybrid outreach. You will collaborate with nephrologists and care teams to support coordinated patient care, use dashboards to track engagement, and document activity in Salesforce or EMR.

Qualifications

• High School diploma or GED required; clinical background preferred.
• Experience with CKD/ESRD and chronic disease management is a plus.
• Ability to educate patients clearly and compassionately in virtual or in-person settings.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients using approved digital platforms.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs.
• Collaborate with nephrologists and care teams for coordinated patient care and referrals.
• Build relationships with nephrologists and staff to support care objectives.
• Document patient interactions and outcomes in Salesforce, EMR, or approved platforms.
• Support referral workflows with patient education and follow-through.
