Job summary

Responsibilities include virtual and in-person interactions, documentation in Salesforce/EMR, and continuous improvement of education materials and workflows. Travel and strong communication are essential.

Qualifications

• Minimum 1–3 years in kidney care or related healthcare fields.
• Experience with CKD/ESRD and dialysis preferred.
• Prior virtual or telephonic patient education preferred.
• Strong communication to explain complex clinical concepts clearly.
• Ability to engage patients remotely and build trust in a virtual environment.
• Organizational and time-management skills for a virtual caseload.
• Proficiency with Salesforce and EMR systems; MS Office experience.
• Valid driver’s license and reliable transportation for local travel.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients on disease progression and treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions.
• Collaborate with nephrologists and care teams for coordinated patient care.
• Build relationships with nephrologists, care coordinators, and staff.
• Partner with internal stakeholders to ensure consistent messaging and workflows.
• Document patient interactions and outcomes in Salesforce/EMR/approved platforms.
• Support referral workflows by educating and following through with patients.
• Utilize dashboards to track engagement and program performance; escalate concerns.
• Stay current on kidney care programs and ensure compliant education.
• Identify barriers to care and connect patients with resources.
• Participate in meetings, training, and performance reviews.
