Job summary

Qualifications

• Has experience guiding CKD/ESRD patients through education and care options.

• Comfortable using CRM and EMR platforms for documentation.

Responsibilities

• Provide virtual care navigation for CKD/ESRD patients and educate on treatment options.
• Conduct scheduled and on-demand patient interactions via video, phone, or messaging.
• Collaborate with nephrologists and care teams for coordinated patient care and referrals.
• Build relationships with clinicians and staff to support value-based care objectives.
• Document patient interactions and outcomes in Salesforce, EMR, or approved systems.
