Job summary

Fresenius Medical Care is seeking a Kidney Care Specialist to support CKD/ESRD patients through education, care navigation, and ongoing support. This field-based role requires residency in or near the assigned territory with local travel to meet patients and nephrology providers.

You will collaborate with nephrologists and care teams to improve outcomes, document interactions, and ensure timely education across the kidney care continuum.

Qualifications

• Minimum 1–3 years of experience in kidney care, chronic disease management, or related fields.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients on progression, treatment options including home modalities, and self-management using approved digital platforms.
