Fresenius Medical Care in the United States seeks a Kidney Care Specialist (KCS) to educate CKD/ESRD patients, drive care navigation, and support informed treatment decisions. This field-based role requires living near the territory and local travel, with possible virtual components.

You will collaborate with nephrologists and care teams, document patient interactions in Salesforce or EMR, track engagement, and help connect patients with resources.

Formación

• High School diploma or GED required; 3–5 years clinical background preferred.
• Strong communication skills for patient education and navigation.
• Experience with CKD/ESRD or chronic disease management is a plus.

Responsabilidades

• Provide virtual care navigation to CKD/ESRD patients.
• Collaborate with nephrologists, advanced practice providers, and care teams.
• Document patient interactions and outcomes in Salesforce/EMR.
• Track patient engagement and program performance using dashboards and reports.
• Support referral workflows and connect patients with resources.
