Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, coordinate care, and support providers in a field-based, virtual, or hybrid capacity. The role emphasizes patient advocacy, relationship building, and timely education across the kidney care continuum.

The position requires travel within the assigned market, strong communication skills, and experience with virtual patient education and CRM/EMR systems.

Qualifications

• 1–3 years in kidney care or related healthcare fields.
• Experience delivering virtual or telephonic patient education.
• Able to travel within assigned market.
• Excellent communication and relationship-building skills.
• Proficient with Microsoft Office and CRM/EMR systems.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients about progression and treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions.
• Collaborate with nephrologists and care teams to support coordinated patient care.
• Build strong relationships with providers and staff to support care objectives.
• Document patient interactions and outcomes in designated systems.
• Support referral workflows and patient education across the kidney care continuum.
• Utilize dashboards and reports to track engagement and program performance.
