Job summary

Qualifications

• High School diploma or G.E.D. required.
• 3-5 years clinical background preferred.
• Licensure or certification preferred, as applicable.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients using approved platforms.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs and barriers.
• Collaborate with nephrologists, advanced practice providers, care teams, and internal partners for coordinated patient care.
• Build and maintain relationships with nephrologists and care staff to support Kidney Care goals.
• Partner with internal stakeholders to ensure consistent messaging and workflows.
• Document patient interactions and outcomes in Salesforce, EMR, or other systems.
• Support referral workflows and patient education across the kidney care continuum.
• Track patient engagement and program performance using dashboards and reports.
