Job summary

The Kidney Care Specialist at Fresenius Medical Care AG supports CKD and ESRD patients through education, care navigation, and ongoing outreach. • Minimum 1–3 years in kidney care, chronic disease management, or related healthcare fields.
• Experience delivering virtual or telephonic patient education is strongly preferred.
• Strong communication skills to explain complex clinical concepts clearly to patients.

Responsibilities

• Provide virtual care navigation and patient interactions (video, phone, or electronic) to assess needs and preferences.
• Collaborate with nephrologists and care teams to support coordinated patient care and referrals.
• Build relationships with clinicians and staff to support Kidney Care objectives and patient follow-up.
