Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients and navigate their care journey in a field-based role. You will travel within the assigned territory to meet patients and nephrology providers, delivering timely education and support to improve outcomes.

The role emphasizes collaboration with nephrologists and care teams, building strong relationships, and using digital platforms to track engagement and outcomes. A passion for patient advocacy is essential.

Qualifikationen

• Minimum 1–3 years of experience in kidney care, chronic disease management, or related healthcare fields.
• Experience delivering virtual or telephonic patient education is strongly preferred.
• Ability to engage patients remotely and build trust in a virtual environment.

• Provide virtual care navigation to CKD/ESRD patients regarding disease progression and treatment options.
• Collaborate with nephrologists, care teams, and internal partners to support referrals and coordinated care.
• Document patient interactions and education in Salesforce, EMR, or approved platforms; monitor engagement and outcomes.
