Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, coordinate care, and support treatment decisions in the field. You will travel locally to meet nephrology providers and patients, delivering education and helping navigate home modalities and care plans.

Ideal candidates have 1–3 years in kidney care, strong communication skills, and comfort with virtual tools, CRM systems, and data-driven workflows.

Qualifications

• High School diploma or GED required; 3–5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.
• Minimum 1–3 years of experience in kidney care, chronic disease management, or related healthcare fields.
• Experience with CKD/ESRD/dialysis populations preferred.
• Strong communication and ability to educate patients remotely in a clear, patient-friendly manner.

Responsabilités

• Provide virtual care navigation to CKD and ESRD patients regarding disease progression, treatment options, and self-management strategies.
• Conduct scheduled or on-demand virtual or in-person patient interactions to assess needs, barriers, and preferences.
• Collaborate with nephrologists, care teams, and internal partners to support coordinated patient care and referrals.
• Build and maintain relationships with nephrologists and care staff to support Kidney Care Advocate objectives.
• Partner with stakeholders to ensure consistent messaging and workflows.
• Provide timely communication and education updates to referring practices.
