Job summary

Fresenius Medical Care seeks a Kidney Care Specialist to educate CKD/ESRD patients and guide them through the care continuum. This field-based role requires travel within the assigned territory and collaboration with nephrology providers to close care gaps and improve outcomes.

Qualifications

• 1-3 years of kidney care experience or related healthcare fields preferred.
• Experience with CKD/ESRD and dialysis populations preferred.
• Strong communication skills to explain complex clinical concepts clearly to patients.
• Ability to engage patients remotely and manage a virtual caseload.
• Proficiency with Microsoft Office applications and CRM/EMR systems.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients regarding disease progression and treatment options.
• Conduct scheduled virtual or in-person patient interactions to assess needs, barriers, and preferences.
• Collaborate with nephrologists, care teams, and internal partners to support coordinated patient care and referrals.
• Build and maintain relationships with nephrologists, care staff, and program leadership to support goals.
• Document patient interactions and education in designated systems (Salesforce/EMR).
• Support referral workflows by educating patients and tracking progress across the continuum.
