This field-based role requires living near the assigned territory and local travel to meet patients and nephrology providers, with opportunities for virtual and hybrid engagement and collaboration with the care team.

Qualifications

• Minimum 1–3 years of experience in kidney care, chronic disease management or related healthcare fields.
• Experience delivering virtual or telephonic patient education is strongly preferred.
• Strong communication and ability to explain complex concepts clearly to patients.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients regarding disease progression, treatment options, and self-management.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess readiness, barriers, and preferences.
• Collaborate with nephrologists and care teams to support coordinated patient care and referrals.
• Build strong relationships with nephrologists, care staff, and internal partners to support Kidney Care goals.
• Document patient interactions, education, and outcomes in Salesforce, EMR, or approved systems.
• Support referral workflows by educating patients and tracking follow-through.
• Utilize dashboards and reports to track patient engagement and program performance.
