Qualifications

• High School diploma or equivalent is required.
• Minimum 1–3 years in kidney care or related healthcare.
• Experience delivering virtual or telephonic patient education preferred.

• Provide virtual care navigation for CKD/ESRD patients and educate on treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs.
