Job summary

Qualifications

• High School diploma or G.E.D. required; clinical background preferred.
• 3–5 years clinical experience in kidney care or related healthcare fields.
• Experience delivering virtual or telephonic patient education is strongly preferred.
• Strong communication and ability to explain complex concepts clearly.

Responsibilities

• Provide virtual care navigation for CKD/ESRD patients.
• Conduct scheduled and on-demand virtual or in-person patient interactions.
• Collaborate with nephrologists and care teams for coordinated patient care.
• Build relationships with nephrologists, clinicians, and staff.
• Support referral workflows and patient education across the care continuum.
• Document patient interactions in Salesforce, EMR, or approved platforms.
• Track patient engagement and program performance with virtual tools.
• Maintain up-to-date knowledge of kidney care and Fresenius Kidney Care programs.
