Job summary

Fresenius Kidney Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, drive early education adoption, and coordinate care across field-based, virtual, and hybrid settings. You will partner with nephrologists and care teams to support timely education and patient engagement.

This role requires strong communication, experience in virtual education, and the ability to travel within the assigned market while delivering high-quality patient education and care navigation.

Qualifications

• High School diploma or G.E.D. required; 3-5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients regarding disease progression and treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs, barriers, and preferences.
• Collaborate with nephrologists, care teams, and internal partners for coordinated care and referrals.
• Build and maintain relationships with nephrologists, advanced practice providers, clinical managers, and staff to support Kidney Care goals.
• Ensure consistent messaging, coordinated workflows, and patient follow-through with internal stakeholders.
• Serve as a trusted resource to referring practices with timely communication and education updates.
• Document patient interactions and outcomes in designated systems (Salesforce, EMR, etc.).
• Support referral workflows to educate, track, and follow through the kidney care continuum.
• Utilize dashboards and reports to track engagement, outcomes, and program performance.
