Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager (RN) to oversee monitoring, follow-up and care management for dually-eligible enrollees with complex medical, behavioral and social needs.

The role focuses on coordinating services and community resources, leading the interdisciplinary care team, and ensuring timely assessments and smooth care transitions to improve outcomes and quality of life. Travel is required to member homes and community sites.

Qualifications

• Associates of Science in nursing from an accredited program required.
• Registered Nurse with ability to independently serve people with complex needs.
• Prior experience in care coordination or dual-eligible populations preferred.
• Medicare/Medicaid knowledge and community resource understanding.

Responsibilities

• Engage enrollees in homes and community settings to build care management relationships.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with providers, community resources, and state agencies to avoid duplications.
• Educate members on benefits, medications, and preventive care.
• Ensure continuity of care after hospitalizations and manage transitions.
