Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts to oversee nurse-led care management for dually-eligible enrollees with complex needs. The role focuses on coordinating health services with community resources to improve outcomes and quality of life.

You will lead the interdisciplinary team, conduct assessments, educate members on benefits, and ensure smooth care transitions. Travel across locations is required, and eligibility depends on licensure in MA.

Qualifications

• Associates of Science in nursing required.
• RN with ability to independently serve people with complex medical, behavioral and social needs.
• Prior experience in care coordination or case management for dual-eligible populations preferred.

Responsibilities

• Engage enrollees in home and community settings to establish complex care management relationships.
• Lead the interdisciplinary care team and collaborate with providers to create holistic care plans.
• Coordinate access to housing, transportation, food assistance, and social services.
• Educate members about benefits under Medicare and Medicaid and support preventive care.
